Here you will complete intake paperwork, set up your method of payment, send secure messages, and schedule appointments. The intake paperwork includes Client Intake Form, Financial Policy, Privacy Policy, No Show Policy, Cancellation Policy and Client Disclosure Release Form. You may be asked to complete additional forms per your therapy treatment plan.
